1. The first step is to open Frontline Education by going to mylearningplan.com and click My Info

2. Click My Evaluations

3. Click Artifacts at the bottom.

4. Click Upload

5. I'm going to walk you through as if I created one. First, Click Name: and enter the text to describe this artifact.

6. Click Description

7. Type a description of the artifact and how it meets an indicator:

8. Click Choose the Artifact Types:

9. Click whatever makes sense for that artifact. In this case I'm choosing student work.

10. Click Categories:

11. Click or check any that apply to this artifact. (In this example I'm choosing Goal Setting)

12. Click the dropdown where it says "Choose a Rubric"

13. Click the indicator that matches your artifact. (My example requires you to chose Instruction)

14. Click Criteria

15. Click whatever ones match your artifact.

16. You can click more than one when you do it..

17. Click Add Criteria

18. Click Rubric if more than one indicator is met through this artifact.

19. Click the appropriate indicator.

21. Click whatever ones match this piece of evidence.

23. Click file-upload IF the evidence is on your desktop or H drive.

24. Then choose the item from your desktop and you'll see it show up where it says File and you can
Click Save You do not need to notify your admin. That's it. You're done.
